Here is one I had that now adapts a Cad-Lasalle trans to an early Buick - 1954 V8 motor. Does not look like a match for your adapter.
Sure looks like a match to me With the exception of the through holes for shift rails and the large cast recess around bearing retainer. All bolt holes and index look the same, slight difference in bearing recess though..

I checked out some of my old speed parts catalogs, and came across one - a 1968 catalog from Radke Auto Parts Speed and Custom in Portland, Oregon. There are two pages listing adaptors for various engine and trans combinations. Using the numbers on your adaptor - 30-37 - I came up with a match. Yours should be a Trans-Adapt adaptor, a common brand back in the day. TD 30-37 fits a 1953-1956 Buick V8 to a 1937-1953 Cad-Lasalle trans. By the way, a guy who went by the Hamb user name of "Heathen" used to have all these answers. Where is he?
