This is the Devil's Elbow Cafe which was located in Devil's Elbow, MO. It was situated in Pulaski county Missouri and was fairly closed to Ft. Leonard Wood. It was on Route 66 until the road was resituated and the building burned in 1974. The term devil's elbow relates to a 180 turn of the Missouri River nearby.
