The 1935 Stubby Stubblefield crash during qualifying for that year's 500. Both Stubblefield and his riding mechanic were killed. Looks like they must have gone over the wall although I didn't realize it until seeing this photo.

 

The second shot looks like Pinebrook Stadium in New Jersey.
