Default Old ET slot wheels, which washers and lug nuts do I need.

I have a pair of old ET slot wheels, I know I need the offset washer for the 5X4.5 bolt pattern, but I see a flat washer and a bevel washer. Do I need the bevel washers and lug nuts or? Here is a pic. Thanks

I have a set of those too. Mine use a beveled washer. Hope that helps. If you know where to find them, I'd like to know.
